PCMI Employee Fingerprint Process

                Thank you for selecting Oak Park School District as one of the Districts in which you would like to substitute teach!  We are looking forward to an exciting school year.
The Michigan State Police has ordered all 3rd party contracted employees to be fingerprinted by the District in which they are working.
          Attached are three forms, each is important and are needed for different reasons:
1.     L-1 Location and Appointment Form:  Must visit: for calling
2.      Live Scan Fingerprint Request (Must take with you to your appointment and/or when on-line or calling to schedule your appointment: Has the agency ID, which is where they send the final results to)
3.     Fingerprint Release Form (If you have been fingerprinted within the last year and there are NO gaps in employment, you must complete form  and email, mail, or bring into district)
4.     The fingerprint fee is $62.00. Please contact L-1 with any questions regarding their payment options.
